Accessing Your Patient Portal (Elation)

 

Our secure patient portal is where we manage communication, track your care, and keep everything organized in one place.

Please note: Access to this portal is only available to active patients of the practice. If you are not yet a patient, you will not be able to create an account.

How to Get Started

 

1. Access the Portal

Click this link to get started on your phone or computer.
 

The mobile app (Elation Passport) is currently available for iPhone. If you’re using a different device, you can still access your account through the link above.

2. Sign In or Create Your Account

  • Use the email address you’ve provided to our office

  • Follow the prompts to create or confirm your password

3. Enable Notifications

When prompted, be sure to allow notifications so you’re alerted when we respond to your messages.

​

4. Start Messaging

Once logged in:

  • Go to your Inbox

  • Tap the compose icon (square with pencil)

  • Select your provider

  • Send your message just like a text

​

You can also attach files, labs, or images directly in the message.

​

When to Use the Portal

Use the portal for:

  • General questions

  • Follow-ups

  • Lab questions or uploads

  • Scheduling requests

  • Non-urgent concerns

When to Call Instead

Please call the office if:

  • You are acutely ill

  • You have an urgent concern​
